Horizon's Bereavement Program
Horizon’s bereavement program engages those individuals who’ve lost a loved one in hospice for 12 months after the patient’s death. The program is multi-faceted and its intent is to ensure that qualified bereavement experts are available to offer support, resources and care to individuals and families who are mourning a significant loss.
Horizon’s bereavement program program strives to:
- Offer support during the grieving process and assist in recognizing the normalcy of the feelings associated with it
- Educate on the grieving process and the impact that a death can have on an individual
- Provide support during the bereavement period and assist in using available resources in the community to help individuals cope with their loss
The bereavement program includes:
- Support from qualified bereavement specialists by phone or through short term counseling for individuals or families
- Information and referral to other resources available in the community if additional follow up or intervention is needed
- Continued assessment for signs of complex grief issues by the hospice team
