Horizon Home Care & Hospice has been making a difference in the lives of patients and families throughout Southeastern Wisconsin since 1990.  Our agency provides well-respected and exceptional care – and that begins with our dedicated Certified Nursing Assistants and Home Health Aides.  Our caregiving staff has the opportunity to make a positive and lasting impact in the lives of their patients and patient families.  In short, our caregivers make patients feel cared for, feel comforted, feel special, and feel home. 

 

Our CNAs/Home Health Aides provide a wide variety of services, those include:

  • Private Duty Care – Provide high level nursing assistant care, companionship, housekeeping and other in-home services that can help a patient maintain an independent lifestyle.
  • Hospice Home Care - Provide priceless comfort, care and life-enriching activities within the home to hospice patients and their families upon a terminal diagnoses.  
  • Hospice Inpatient Care – Provide compassionate hospice care to patients with life-limiting diagnosis in a home-like setting.
  • Home Care – Be part of the rehabilitation services provided by our Home Care Team by assisting patients with bathing, grooming, and other personal care needs. 

 

As part of an interdisciplinary team, our CNAs/Home Health Aides serve patients throughout Milwaukee, Waukesha, Ozaukee and Washington Counties (mileage reimbursed between visits).

 

Full and Part-time opportunities available on primarily 1st and 3rd shift with patient cases/shifts ranging between 1-12 hours in length.

 

Beyond a positive attitude and a desire to truly make a difference in someone's life, you must hold a valid Wisconsin CNA licensure, valid WI driver’s license, and a reliable, insured automobile.
